The Baxter’s neighbor Chuck Larabee stops by with his German Shepard, Lady, to let them know that their tiny dog, Muffin, has impregnated his dog. Chuck is very selective about who Lady breeds with because purebred German Shepard puppies are in demand, and he’s not pleased at the litter of mutts Muffin has caused. Meanwhile, Eve takes the highroad and leaves a note on the parked car that Mandy hit, but somehow Mandy reaps the benefits of her sister’s integrity.

While Greene tries to distract himself from his worsening brain tumor by working hard, he reluctantly recognizes that his debilitating illness is hampering his ability to perform the job. Watching other terminally ill patients struggle with their treatments also has a sobering effect on him. Greene has a poignant reunion with his ex-wife, Jenn, and mediates between the staff and a stubborn but deluded homeless man, Al Ervin. Confident, young first-year intern Gregory Pratt, who has transferred from another hospital, hopes to become a resident at County General soon--but first he must heed advice from Greene about his bedside manner as they treat a family man, Stan Seidel who receives the terrible news that he has advanced prostate cancer.

Manny Soleimani is found dead in his bedroom, the morning after he married a 25-year old woman. Manny was old enough to be her grandfather. His young wife, Yasmin, doesn't want the M.E. to touch the body, but the victim's 40-year-old son, Ben, who called 911, wants an autopsy. After a hearing in front of a judge, Briscoe and Green get their way. Since the couple was married on a Saturday night and their marriage certificate hadn't been filed yet, they allow the investigation. The M.E. says Manny was murdered, discovering he'd been smothered to death. Manny was a caviar magnate and Yasmin's family was his biggest competitor. They think it might be business related, since Yasmin had an alibi and there was also a prenuptial agreement, she clearly didn't kill her husband.

While Nina and Faheen are brought together, Jack and Michelle must try and decipher their conversation in Arabic for any clue to where the bomb is located; Kim and Miguel are questioned by the police for details about a grizzly discovery in the trunk of Gary's car; and Tony interrogates Reza and Bob, who contradict each other about their possible terrorist ties.

An armored car robbery kills a guard and a body is found in a trash truck.
