Brennan and Booth are called to the site of an explosion in a Miami hotel where a young woman has been burned to death. Initially, the explosion is thought to be the work of a Colombian drug cartel intending to target Judge Dolores Ramos, the Colombian judicial representative. State Department official Alex Radziwill insists everything to do with Judge Ramos and the case go through him, which becomes both annoying and problematic when he stands in Brennan and Booth’s way as they do their jobs in pursuit of key information about the case. When fingers point to Ramos’ husband and son, Booth and Brennan are shocked to learn who was behind the murderous plot, and how far the State Department will go to protect a murder suspect.

Pediatrician Gilbert Keppler (Guest Star Lawrence Arancio) is found guilty of sexually assaulting four of his male patients. Soon after his trial, his attorney, Lionel Granger (Guest Star David Thornton), informs the detectives that Keppler will be suing the NYPD and the Denslow Medical Center on the grounds that they knew he was sexually abusing patients long before they took any action to stop him. Detectives Elliot Stabler (Christopher Meloni) and Olivia Benson (Mariska Hargitay) decide to go directly to Keppler for questioning only to find him murdered in his apartment. To the detectives surprise, ADA Alexandra Cabot (Guest Star Stephanie March) shows up at the crime scene, after three years in the witness protection program. Attorney Lorna Scarry (Guest Star Mariette Hartley) reminds Cabot how tough solving a case can be. The SVU team must put the past and lost time behind them in order to solve the case. When a 16-year-old girl who had received a kidney transplant from her father arrives at the emergency room feeling dizzy, Carter traces the problem to a new drug that the girl was taking to control seizures related to her kidney dysfunction. Her father feels guilty because he pushed for her to take the new drug, and its side effects may force his daughter to go back on dialysis. The doctor who prescribed the medication, Dr. George Henry (Chad Lowe - "Life Goes On"), is Carter's former student. Carter asks Neela to post information about the drug's side effects on the Internet, which causes an uproar when the pharmaceutical company finds out. After Abby and Neela complain about their lackluster dating lives, they are assigned to work with Jake (recurring guest star Eion Bailey - "Almost Famous," "Band of Brothers") and Kovac--men they each find attractive.
