Dr. Rhodes (Colin Donnell) begins his fellowship in cardiothoracic surgery, but gets off on the wrong foot with his challenging new boss, Dr. Latham (guest star Ato Essandoh). Dr. Choi (Brian Tee) navigates life as the new chief resident and finds the road a bit rocky, while Dr. Charles (Oliver Platt) approaches Sarah (Rachel DiPillo) with an interesting opportunity that catches her off-guard. Elsewhere, April (Yaya DaCosta) works to come to terms with her recently discovered illness and Dr. Manning (Torrey DeVitto) deals with a complicated case involving a pregnant woman who was a victim in a high speed collision. Meanwhile, Dr. Halstead (Nick Gehlfuss) takes exception to the actions of fourth-year medical student Jeff Clarke (guest star Jeff Hephner), whose past history with Dr. Manning may be a factor. Sharon Goodwin (S. Epatha Merkerson) tries to pick up the pieces and move on with her life in the aftermath of Bert leaving. Marlyne Barrett also stars.

Dr. Halstead (Nick Gehlfuss) and Sharon Goodwin (S. Epatha Merkerson) deal with a husband intent on harvesting his recently deceased wife's eggs, but soon discover some vital information that surprises everyone. Dr. Charles (Oliver Platt) is brought in by Dr. Choi (Brian Tee) to aid a patient who is trying to remove his own arm from his body because he feels like it doesn't belong. Nurse Sexton (Yaya DaCosta) befriends the father of a child who is rushed into the hospital after swallowing magnets - a situation that Dr. Rhodes (Colin Donnell) and Dr. Manning (Torrey DeVitto) are concerned could present dangerous complications. Meanwhile, Sarah Reese (Rachel DiPillo) finally learns about where she will do her residency, but has last-minute second thoughts.

An erratic patient comes to the emergency department claiming that his heart is pounding inside his head and takes drastic measures to stop it, leaving Dr. Choi (Brian Tee) and Dr. Charles (Oliver Platt) to figure out the best course of action. Dr. Manning (Torrey DeVitto) works on a young boy with a broken wrist, but her suspicions abound when the facts don't add up, prompting her to report the situation to Goodwin (S. Epatha Merkerson) and Dr. Charles, who must make a serious decision. Dr. Rhodes (Colin Donnell) impresses Dr. Downey (guest star Gregg Henry) with his skills inside the operating room, but the pressure takes a toll. Elsewhere, Will (Nick Gehlfuss) receives news about the pending lawsuit, April (Yaya DaCosta) feels a spark in her social life and Sarah (Rachel DiPillo) tries to determine the cause of her recent anxiety. Marlyne Barrett also stars.

Meanwhile, at the urging of Dr. Charles (Oliver Platt), Dr. Choi (Brian Tee) turns to an unlikely source for help. Marlyne Barrett also stars.

Dr. Manning (Torrey DeVitto) and Dr. Charles (Oliver Platt) deal with a 16-year-old patient suffering from abdominal pain who adamantly refuses any drugs for fear of winding up like her drug-addicted father. Dr. Halstead (Nick Gehlfuss) relives a bit of his past when a patient, along with his brother, turn out to be former high school classmates whose past history with him is anything but stellar. Meanwhile, Dr. Choi (Brian Tee) works on a woman 32 weeks pregnant who has a big reason to do everything in her power not to deliver the child, much to the dismay of Goodwin (S. Epatha Merkerson), Charles and the hospital staff. Elsewhere, April (Yaya DaCosta) is less than thrilled to discover her brother Noah (guest star Roland Buck III) has an enterprising side-business that involves her new boyfriend Tate (guest star Deron J. Powell). Dr. Rhodes (Colin Donnell) avoids meeting up with his sister Claire (guest star Christina Brucato) so he doesn't have to deal with family issues. Rachel DiPillo and Marlyne Barrett also star.
